Accepting new patients
Yes, Citizens Health Corporation is accepting new Medical, Dental and Behavioral Health patients at Citizens Health Center and Barton Annex Health Center. Citizens Health Center located at 1650 N. College Ave, Indianapolis, In., and Barton Annex Health Center located at 501 East Street, Indianapolis, In.

What information do I need to bring?
For all new patients you must bring the following documents:
- Picture ID (for those who have no picture ID we will take your picture)
- Social Security Card (if you have one)
- If you are uninsured proof of Marion County, Indiana residency is required for certain fee discounts (utility bill or similar document)
- If you have insurance please bring proof of all Medical and Dental Insurance(s) which you may have
If you are uninsured, please bring 3 consecutive pay stubs, or a current unemployment statement, or a recent tax return, or a letter from your current employer (on company letterhead) indicating that you are employed but paid in cash. If unemployed, please bring a letter from the Indiana Workforce Office stating you are not working or a notarized letter from a community organization stating you are not employed.
- Children needing immunizations must have a current record of immunization received.
If you are bringing a child in for an appointment and you are not their legal guardian or parent, please call the eligibility office at 317-924-0243 to find out what additional information you will be required to bring in.
All patient information is updated at each visit. Financial updates are done once a year

I do not have any insurance can I still be seen?
Yes, Citizens offers a sliding fee discount for people who are uninsured. However, you must bring proof of total family income and provide the total number of people who are dependent on that income. Without this information you will be responsible for the full amount of the visit. The discounts are based on your income and if your income is higher than 200% of the federal poverty income guidelines you cannot receive a discount. Please call one of our offices to get additional information.

How much does an office visit cost?
For our insured patients we accept your insurance co-pay and will bill your Insurance Carrier.
For uninsured patients office fees are based proof of the patients’ total family income and the number of people living in the household who are dependant upon that income. However, this offer is for patients who are at or below 200% of US Dept. of Health and Human Services annual Federal Poverty Income Guidelines. However, ALL laboratory test(s), Podiatry services, Optometry services are not included.
For our uninsured patients whose annual income is greater than or at 201% of the US Department of Health and Human Services Federal Poverty Income Guidelines there is no adjusted of charges based on income.
For our Medicare patients the twenty percent cost can be reduced with annual proof of Income and the provision of the number of people dependent on that income. For those individuals who do not provide proof of income or insurance information you will be charged full price.

Do you accept Medicaid, Medicare and/or Private Insurance?
Yes. Citizens Health Corporation is contracted with al three Medicaid plans, Citizens also accepts Medicare. The following Commercial Insurances are accepted, however, if your insurance company name does not appear on our list please call 317-396-0303 as we are always adding new commercial insurance carriers.

Do you offer on-site laboratory services?
Yes. We have an onsite laboratory which does testing of Health Center patients only. Laboratory tests are not discounted. If you are uninsured you will need to pay for the laboratory test prior to having the test ran. Consult with your provider regarding the cost.
Do you have payment plans?
Yes. Citizens Health Corporation will work with you in the development of a payment plan that will fit your budget. However, the following services must be paid for PRIOR to receiving the service:
Dentures (Full or partial)
Root Canals
Crowns
OB/GYN Ultrasounds
Cryo’s, Colpo’s, Leep’s
In house procedures such but not limited to toe nail removal, wart and mole removel, etc
Podiatry items such as boots/shoes, etc.

Do you have X- rays on site?
No. Citizens’ refers all patients who are in need of X-rays to either Wishard Hospital or to the hospital your insurance indicates you must go to. Citizens where possible will try to accommodate those demands but it is the patients responsibility to make sure that their insurance is accepted at all Indianapolis Hospitals. Citizens/Barton will make the referral appointment for the x-ray test(s), however Citizens will not pay for the x-rays which Citizens/Barton Providers have order.
Will Citizens make a referral appointment for me?
Yes. If a Citizens Provider request that a patient be sent to a specialist(s) Citizens will make every attempt to accommodate your busy schedule. Again, for our insured patients it is your responsibility to verify with the specialist that they will accept your insurance. For our uninsured patients, any referral made on your behalf is your financial responsibility and not Citizens.
What referral services does Citizens provide?
Yes. For our patients and if a Provider Citizens make every attempt to accommodate your busy schedule
However, any referrals that are made on your behalf are your financial responsibility and not Citizens.
Referrals are made for existing Citizens Health Corporation and Barton Annex Health Center patients depending on your providers orders.

Do you take walk-in patients?
Yes. Citizens Medical Department has walk-in hours Monday through Friday from 7:30 am to 4:00 pm and on Saturdays from 9 am to 1 pm. The Dental Clinic accepts limited emergency only walk-in patients please call 317-396-0278 for more details.

Are you open on Saturdays and what services do you provide?
Yes. Citizens’ is opened on Saturdays from 9 am to 1 pm for Medical only patients. Dental, Social Services and WIC are closed on Saturdays.
The Saturday Clinic is open on a first come first served basis (walk-in). The focus of the Saturday Medical Clinic is on patients who are sick on that day all other patients will be worked in as time is available.
Laboratory services are available on site during the Saturday Clinic.

HIV/AIDS
The Citizens Health Corporation offer confidential HIV testing, as well as education and prevention services for the general public, especially targeting at-risk populations.For more information cal 317-924-6351. All patients with a positive HIV/AIDS test result are referred to Methodist Life Care, Wishard Infectious Disease or to the Damien Center for care coordination.
Do you provide sexually transmitted disease (STD) care and treatment?
Yes. Citizens’ does test and treat for certain STD’s, other are referred to the Marion County Health Department.
Do you provide testing and treatment of HIV/AIDS?
Citizens test for all STD’s. Citizens provide for pre and post test counseling, as well as does the blood drawing for the testing. If the test is positive Citizens does not provide treatment for HIV/AIDS, instead Citizens refers patients with a positive test result to the Marion county Health Department for further testing and treatment

Medical (Non-OB/GYN)
Established patients should be able to see a provider the same day if they call or walk in between 7:30 am and 4:00 pm.
If a patients wants to see a particular provider who does not take walk-patients you should be able to get an appointment within the week however not all providers are available at the Health Center everyday in which case the appointment time maybe a little longer.
Medical (OB/GYN)
Established emergency patients will be seen as soon as is appropriate. New Medicaid OB/GYN patients are seen in accordance with the individual Medicaid plan regulations.

Dental (Non emergency)
New patients are accepted on Mondays only with an appointment given within one week, depending upon available appointments.
Established patients should be able to be seen in the Dental Clinic within three days depending upon available appointments
Dental (Emergency)
The Dental Clinic accepts Dental emergency walk-in patients on a first-come, first-serve basis at 8:30 am and again at 1:00 pm. However, the number of emergency patients that can be accepted on a daily basis depends on the number of available time slots.

Women, Infants and Children (WIC)
Marion County Health Department operates numerous WIC programs throughout Marion County, Indiana. One of its WIC programs is the Citizens WIC program located within Citizens Health Center, 1650 N. College Ave, Indianapolis, In. 46202.
What is WIC?
WIC is a supplemental Nutrition Education Program for Women, Infants and Children
What does WIC provide?
Nutrition and health education to help families eat well and be healthy
Personal counseling about nutrition
Breastfeeding support and assistance
Referral to health care and other kinds of help for families
Special healthy foods vouchers
Breastfeeding Support and Promotion
The Citizens WIC program recommends exclusive breastfeeding for the first 6 months of life and continued breastfeeding until at least 1 year of age. When circumstances warrant, the second best choice is to encourage moms to pump mother’s milk and feed breast milk in a bottle. The third choice would be formula feeding.
Who qualified for WIC?
Women who are pregnant, breastfeeding or recently had a baby
Infants under 12 months old
Children under 5 years of age
Other WIC requirements?
Applicants must be “at nutritional risk.” The specific criteria and parameters that indicate risk are determined by a health and dietary assessment performed by a health professional at the time of certification.
Applicants must be “income” eligible. Families receiving Medicaid, Food Stamps, and/or TANF are income eligible for the Indiana WIC Program. If you are not a member of these other programs, check the income chart below to see if your family might qualify.

If you are pregnant, count yourself as two (2). For households with more than 12 members, add $6,660 for each additional member. * Household means a group of people (related or not) who are living as one economic unit.

Healthy Start Program
There are three main services offered by Indianapolis Healthy Start:
Case Management:
- One on one support for pregnant women from birth until the child is two years of age.
- Provide in-home services when needed
- Information and referrals for community and medical resources
- Assist with tips for dealing with stress
- Resources for dads
Health Education:
Provide information to improve the health of families before, during and after pregnancy
Infant safety and parenting classes
Additional educational workshops are offered in clinics, schools, churches, community centers and other locations
All classes are free and pen to the public
Outreach:
- Increase community awareness about problems and solutions regarding infant health
- Encourage pregnant women to get care as soon as they know they are pregnant
- Volunteer opportunities available to assist in saving the lives of Marion County infants
